1. Enrollment Confirmation - You must return this form by fax or mail as soon as possible to complete your registration for Tufts University Summer English Language Programs. Be certain to also sign the Terms and Conditions of Enrollment page. Students under 18 years of age must also have the signature of parent or guardian.
    
2. Terms and Conditions of Enrollment - Please read this material carefully. The rules and regulations below are strictly enforced. After signing this form, make a copy for your reference and return the original to the Summer English Language Programs office.

3. How to Pay Your Tuition - Your tuition and fees must be paid in advance. Please note on the Enrollment Confirmation Form above how you intend to pay your bill.
    
4. How to Request an Airport Transfer - Our program staff will be pleased to meet you at Boston's Logan International Airport if you arrive within one day of the start of the program. This service is free of charge if you provide the following information at least 10 days before your arrival.

5. Immunization Record - Students who attend any Tufts education program are required to comply with the Massachusetts Immunization Regulations and demonstrate immunity to certain communicable diseases.
    
6. Health Form - In order for the staff clinicians at the Tufts University Health Service, or any other medical facility, to render services to you during your stay at Tufts, it is necessary for you (or your parent/guardian if under 18 years of age) to complete this consent form and short health history.
    
7. Overnight Permission Form - Our programs provide students with a living situation similar to that of a first-year university student. Students are expected to manage their own time, choose their own activities, and behave responsibly without extensive adult supervision. Students under 18 years of age may leave campus overnight on Fridays and Saturdays – with a chaperone 26 years of age or older and one familiar to the family - if they have written parental permission on file and complete an Overnight Request Form at least 2 days prior to their expected departure. This form is also available at the residence hall reception desk and must be submitted to the Residence Life Staff for approval. Any student who fails to comply with the overnight sign out policy will be subject to discipline and possible termination from the program.

10. Summer Institute Handbook - A complete handbook for students enrolled in the six-week intensive English language and study skills course for intermediate and advanced students planning to attend a graduate program in the United States or Canada.
